Pathway to Home Model

The Pathway to Home model has three elements to its structure, namely:

  1. Interventions and services that prevent homelessness
  2. Temporary accommodation and homeless services
  3. Housing with supports

The outcomes of the Pathway to Home model are as follows:

  1. Effective early interventions and assistance that diverts a person or household from homelessness
  2. When homelessness does occur, the Pathway to Home model will encure a person's progression towards an exit from homelessness via temporary accommodation into a long-term housing option
  3. Therein a person resides as a tenant with a tenancy agreement, either with visiting housing-related support services as required or on-site housing related support services
  4. The person is thereby supported and empowered to live independently and successfully as a full participant in the community and society.
